This form can be completed on-line and e-mailed direct to the Norwich Union MID Helpdesk oralternatively you can print a Microsoft Word version of the form send to the Norwich Union MID Helpdesk via:

Once the form has been received, contact will be made by the Norwich Union MID Helpdesk who will confirm the User Identification Details and discuss the best method for sending fleet details to the MID.
